| At this point this is the paperwork needed (this may change): One List of coaches and all players in alpha ( To use as a check list) Two - Copies of each 1601 paper clipped together in alpha order (Signed In Blue Ink) Two - Copies of each 1628 (Signed In Blue Ink) Two - Photos for each player and adult on the roster (Must Be) 1 x 1-1/4 (Head Shot) Two - copies of all new updated licenses (If not already emailed to Lydia) One - copy of Birth docs for all new and any unverified players (Only) |

| Team paperwork no longer needs to be given to Lydia in a binder. Each team's paperwork should be placed in an envelope with the Club name, Coach, team name, team number use same as last year written on the outside of the envelope. All paperwork is processed in the order that it is received. If a team does not have the minimum number of players, I will not be able to accept the team. Also, if the team is missing paperwork and/or needs new photos, the paperwork will be returned and you must come pick it up at Lydia's house. (2889 Freedom Blvd Corralitos, CA 95076) | |
| Once your team is approved Lydia will contact your team manager by email when everything is ready for pick up. All paperwork will then need to be put into a binder using a plastic sleeve per player. Please make sure that your coaches know that they should not have any birth docs in their binders. | |
